Yamaha PA-10B AC Power Adapter Recall

In conjunction with the U.S. Consumer Product Safety Commission (CPSC), Yamaha Corporation of America is voluntarily recalling its PA-10B AC Power Adapter with date codes ranging from 2126 to 2319.

Yamaha is printed on the front of the black power adapter. The date code is printed on the back.

Yamaha's PA-10B AC Power Adapter was bundled with five Yamaha analog mixing console models – MG10, MG10X CV, MG10XU, MG10XUF, and MG12XUK – and sold as a stand-alone replacement part.

Only Yamaha's PA-10B AC Power Adapter is being recalled.

Consumers should immediately stop using the PA-10B AC Power Adapter and contact Yamaha for a free replacement.

Yamaha will provide a free replacement adapter to any consumer who purchased a Yamaha mixing console that contained a defective PA-10B AC Power Adapter and/or purchased a defective PA-10B AC Power Adapter as a stand-alone replacement part.

  • Confirm the model name of the adapter is PA-10B, as shown below.
  • Confirm that the date code on the adapter is 2126, 2319, or any number between 2126 and 2319.
  • If your Yamaha power adapter is not a PA-10B model with a date code ranging from 2126 to 2319, your power adapter is not part of this voluntary recall. You may continue to use it with the appropriate Yamaha product.
  • If your Yamaha power adapter is a PA-10B model with a date code ranging from 2126 to 2319, please follow the steps below to get your free replacement.

Destroy the Affected PA-10B AC Power Adapter

After validating that you have an affected PA-10B AC Power Adapter, we ask that you destroy the PA-10B AC Power Adapter, as outlined below, and submit pictures providing proof of destruction to Yamaha along with your claim:

  • Unplug the PA-10B AC Power Adapter from the electrical outlet.
  • Disconnect the PA-10B AC Power Adapter from the MG series mixing console.

DO NOT PROCEED TO THE NEXT STEP UNTIL BOTH OF THE ABOVE HAVE BEEN COMPLETED.

  • Cut the affected PA-10B AC Power Adapter cable close to the brick.

  • Take a picture of your PA-10B AC Power Adapter, label side up – showing the model number, with the cable cut off.
  • Take a picture of your PA-10B AC Power Adapter, label side down – showing the date code, with the cable cut off.
  • Include BOTH images with your claim submission to Yamaha.
  • Dispose of or recycle the destroyed PA-10B AC Power Adapter and cable in compliance with your local regulations.
